RECRUITMENT PROCESS CONSENT FORM

Dear Applicant,

Thank you for taking an interest and for applying to our company. Your privacy is important to us, and this consent form provides information about our use and disclosure of personal information collected during the recruitment process.

Throughout the recruitment process, we collect necessary personal information about you as part of assessing your suitability for the position. If you are unable to provide us with accurate personal information and consent, we will not be able to proceed with your application.

We only gather and process personal information about you that is relevant to any or all of the following recruitment procedures: job interviews and assessment, job profiling and project matching, and background or reference checks, pre-employment activities. To help us evaluate your application for employment purposes, our Recruitment staff will collect these types of personal information through electronic and/or paper-based forms:

  1. Name, Identity, and Contact details such as your full name, birth date, citizenship, gender, height, residence address (present/permanent), contact numbers (landline and/or mobile), personal email address, and professional licenses.
  2. Professional and Education Credentials include your highest educational background, academic transcript, certificates, professional certificates and/or licenses.
  3. Employment History includes details of your employment background, including position title and rank, employment duration, employment clearance/status, duties and responsibilities, employment references, and salary/compensation.

Please note that the information you provide during the recruitment process may be shared with our project management team and/or a third party strictly for validation procedures only as listed above. Data sharing with third parties shall conform with regulations by the National Privacy Commission.

In processing your personal information, we maintain appropriate measures designed to protect its security and confidentiality. We process applicant personal information using the following facilities: online job ports such as Monster, Jobstreet, applicant tracking system of the company, cloud storage, company email (ipiphil.com), company website (www.ipiphil.com), social media accounts such as Facebook (https://www.facebook.com/ipirecruitment/), LinkedIn (https://www.linkedin.com/company/information-professionals-inc-), Instagram (@informationprofessionalsinc).

Your applicant information is retained with us for five (5) years after the release of the final application result, after which, paper documents and electronic files pertaining to your personal information will be deleted or removed from our system in compliance with guidelines under the Data Privacy Act. If you are hired by our company, your application information and other recruitment process information will be forwarded to the HR department as part of personnel records which will be retained for five (5) years after the end of employment.
